WRITER WAS DRAWING BLOOD FOR A PATIENT LABEL REQUISITION. AFTER THE DRAW WAS COMPLETED WRITER SLID THE SAFETY UP OVER THE NEEDLE AND PROCEEDED TO WALK TOWARDS THE SHARPS CONTAINER FOR DISPOSAL. AS WRITER PUSHED THE BUTTERFLY NEEDLE into sharps box the safety slid back and poked writer in finger. Safety was not clicked safely in place
