On [REDACTED] [REDACTED] approx. [REDACTED] reg clerk entered the freight elevator #2 to check a pallet of parcels at [REDACTED] on the 2nd floor. As she entered the elevator the door came down and hit on her head. I conducted the investigation and shut down the elevator reported to [REDACTED] ref.# 76096859. she is claiming pain on her neck and went to emergency for medical treatment. She was released from the hospital.
