ACCORDING TO EMPLOYEE #1 HE WAS USING THE YELLOW PALLET JACK (NO 1041) AND FEEL SOMETHING UNDER THE EQUIPMENT. HE AND COWORKER TRYING TO SOLVE THE ISSUE BUT THE PALLET JACK MOVE TO THE SIDE CREATING A WOUND IN THE RIGHT THUMB OF THE COWORKER. I WAS INSIDE THE BUILDING AND SEE THE SITUATION IN THE DISTANCE AND RUN TO THE AREA FINDING THE ISSUE. WE PROCEED TO HELP OUR COLLEAGUE AND CALL [REDACTED].
