On [REDACTED] at [REDACTED] I was getting off the back of the [REDACTED] [REDACTED] when my foot slipped. After my foot slipped I proceeded to fall. I tried to brace myself and put my hand under me and my back hit the back of the [REDACTED]. Now my whole right side of my body is in pain. Also I have a headache.
