What happened

On [REDACTED] Miami Logistics Motor Vehicle Operator was at a Happily Chic located at [REDACTED] he was opening the truck liftgate to load up mail from the firm. The rear liftgate caught Employees finger from his left-hand ring finger as he was trying to release the locking chain. He was wearing safety gloves when he was operating the lift. He called the office and reported the accident. When employee came to the office I had him fill out a [REDACTED]. I sent him to MD Now Urgent Care located at [REDACTED]. [REDACTED]
