on [REDACTED] around [REDACTED] I was loading my [REDACTED] into my hamper. As I was picking up the tub of ADVOS i received a sharp pain run through my back down my left leg. I quickly sat the tub down and asked for assistance. I went to tell supervisor [REDACTED] that I was unable to carry any overtime because I Had just hurt my back and that it was hurting really bad.
