Before the incident
Walking along the platform to dock steps to climb down and get into his truck.
What happened
On [REDACTED] [REDACTED] Logistics Motor Vehicle Operator was walking West on the [REDACTED] platform at [REDACTED] P&[REDACTED] plant. When passing stall 27 he slipped and fell on wet cardboard boxes along the floor. 2 other Motor Vehicle Operators were in the area and immediately assisted Employee. As they were lifting him up off the ground Logistics supervisors walked over to assess the situation. He stated that he felt pain in his right shoulder. Pictures were taken and the injury report started. The area had damaged gaylord boxes staged for disposal and it was heavy rains today. There were no workers on the platform clearing area therefore it was built up and the water from the rain caused the boxes to fall and spread across the ground. Employees were walking over the area were doing the same when the incident happened. This combination created an unsafe work condition near the ramp by stall 27.
