On [REDACTED] at [REDACTED] [REDACTED] claims that she was hit in the head with the top shelf of an Apc. [REDACTED] mentioned the accident [REDACTED] on [REDACTED] at [REDACTED]. I asked the employee why did she fail to report an accident when it occurred. She said she continued to work. She also claims that she had a witness [REDACTED] who 's a contract driver. I asked Miss [REDACTED] where is the APC that she was struck with. She said she doesn 't recall. I asked her was the safety latch attached to the APC she said she didn 't recall.
