On [REDACTED] [REDACTED] the custodian came to see the supervisor. He said that he injured his foot and took supervisor to custodians room to explain of how the accident happened. A metal bar fell off the back end of the workbench and the front end of the bar landed on the side of the vice fell off the end of the vice and landed on his left big toe.
