CONTINUED WITH DUTIES AND DID NOT REPORT ACCIDENT TO SUPERVISOR ON TOUR NOR WERE THERE ANY WITNESSES. WOKE UP ON [REDACTED] WITH [REDACTED]. REQUESTED A DOCTOR 'S VISIT.
