Before the incident

Employee was taking out a staged job to the cell floor and when she returned she was carrying a part in her hand and failed to see the tote on the floor.

What happened

Employee was walking through the east side gate entrance of our storeroom department near [REDACTED] and a heavy tote with material was sitting on the floor next to the entrance. Employee walked through the gate entrance and didn 't notice tot on the floor and ended up hitting short side of the tote with her lower left leg.
