An employee was performing a routine job of installing the [REDACTED] roll cart tubes into the machine.

What happened

An employee was performing a routine job of installing the [REDACTED] roll cart tubes into the machine. Once the tubes are installed anchors are secured into place to hold the tubes stationary. One of the anchors was broke and therefore did not retain itself on the cart. Therefore this anchor was laid on the roll cart for future use. The operator leaned over the roll cart to pull the anchor into an area reachable by the crane at which point he felt pain in his lower back hip area.
