Before the incident

From dates [REDACTED] the ultrasound department did not have an evening technologist. During this time I worked extra hours to cover the evening shift and weekend shifts that were the responsibilities of the even technologist. On [REDACTED] I visited my NP [REDACTED] for right sided cervical radiculopathy. AT that time I was prescribed a muscle relaxer and NSAID to help with th epain. I;ve been dealing with that pain intermittently since and combat symptoms
