Before the incident

EE was working with another EE to remove a shaft from the rollstand They evaluated several methods and decided the safest way was to hold a hammer on the end of the shaft and strike the head of that hammer with a deadblow sledge hammer which should have kept his hand out of the line of fire The sledge hammer accidentally struck the side of the equipment as it was being swung which changed its path so it struck the hammer being held and pushed the EEs hand into the plate
