Before the incident

Employee was hooking up a 20 foot dolly to the tug to move it out to the ramp.

What happened

The tow bar on the dolly was stuck and employee could not lift it up to hook it unto the tug. Another foreman used a forklift to lift the tow bar when it flew up and struck him on the forehead

EE suffered a laceration to the forehead that required 5 stitches
