What happened

On [REDACTED] EE lifted a gas generator with a co-worker when the part slipped in EE 's hands. The generator bumped against his head and the weight of it pushed him back against a large box. The employee 's shift was over at [REDACTED] but the supervisor was not notified of the injury until [REDACTED]. On [REDACTED] the supervisor was notified that the employee would not be in for his scheduled shift due to pain in his neck back and knees. He also stated that he would like to have an incident report recorded. The supervisor then requested a written statement of facts regarding the injury.
