WHILE EMPLOYEE WAS ATTEMPTING TO USE NEW DIE GRINDER THE CUTTING WHEEL CAME APART AND STRUCK HIM IN THE FACE. THE EMPLOYEE RECEIVED A CUT TO THE SIDE OF HIS LEFT EYE AND ALSO EYE WAS DAMAGED AND BLEEDING TO THE POINT THAT HE COULD NOT SEE CLEARLY.
