What happened

Operator was loading empty kegs when one got jammed up on a dented roller. Operator tried to continue pushing it forward but the keg tipped over due to not being able to make it passed the roller and rotated while they were holding it. While it was twisting their hand got stuck in the handle and twisted with the keg.
