Before the incident

[REDACTED] Driver was filling an air bag on a reefer trailer. As the team member was exiting the trailer the team member was looking back towards the pallet to ensure that the air hose was not getting stuck on the pallet rather than looking in the direction that the team member was walking.

What happened

The team member stepped on the air hose causing the team members ankle to twist and their foot fell into the grooves of the floor on the trailer.
