Before the incident

Using slide board to move pt back into bed

What happened

Nurse and Radiology tech using slide board to place pt back in bed. Nurse was pulling and radiology tech was pushing We moved pt on the count of 3 and we moved pt half way and counted again and moved the rest of the way. When I went move pt over the second time I felt my back pop and my right leg went numb and I couldn 't stand up straight and was having a hard time putting weight on right leg.
