What happened

While taking a skid off of the up ender the employee doing line service accidentally got the band from the skid caught on the base it was on. When the employee reversed out the skid along with the base came forward flinging the glass from the skid onto the forklift. Most of the glass hit the forklift but some glass did land and hit the employee on the forklift.
