Retrieving cases of rolls from top shelf of Bakery freezer racking while standing on a mobile ladder.

What happened

Employee was up on the ladder getting a case of rolls down and there was a lip on the edge of the shelf. She was having trouble getting the box over the ledge. When she tried to lift it she slipped and fell of the ladder.
