Before the incident

Employee was going to grab boxes off of a rug rack that had a pallet on the floor in front of it.

What happened

After the employee grabbed the box she backed up and placed her right foot on the pallet on the floor. In doing so her foot slipped and went in-between the pallet boards. At that time the employee tripped and fell to the floor hurting her right ankle.
