Before the incident

Installing a metal gasket on engine.

What happened

The metal gasket bolt hole was undersized. Employee could not find the correct tool to drill out the hole to increase the size. Employee put the gasket on the drill press and thought he could hold it in place while opening up the hole. The drill bit grabbed the metal gasket twisting it in his had. The metal gasket lacerated the hand through his glove.
