What happened

Employee was pulling stacks of empty boxes off a pallet and placing them under chutes as needed. One stack of empty boxes fell over and she tried to catch it before it hit the ground and in doing so she clipped (or stepped on) the pallet they were on and fell on the floor. While falling she tried to break her fall with her right arm and landed hard on the floor (on her right hand wrist).
