What happened

Employee was backing out of trailer when the force of the forklift reversal caused the trailer to roll forward just as the forklift was crossing the dock door threshold. The forklift fell rear-first onto the ground outside and then as the trailer continued to roll the front end of the forklift fell forward as well.
