The employee was swaping seamers and was trying to get the disconnect to connect with the fitting.

The employee was swapping seamers on Line 2. While attempting to remove a quarter inch push-to-connect air line fitting the threaded portion of the fitting broke loose. The millwright 's left index finger contacted the edge of the air cylinder body causing a laceration.

The tubing did not discconect correctly causing the air cylinder frame to catch and lacerate the employee 's left index finger.
