Before the incident

EE was loading a trailer at dock door 10 using a Fork Lift. EE was nearing completion and placing one of 3 pallets still required to complete the load.

What happened

Driver of Truck Trailer began to pull off as the EE was crossing the threshold of the dock door into the trailer. Forklift fell through opening EE jerked up front and back as lift contacted the ground outside of the loading dock
