What happened

While Mr. [REDACTED] was on forklift [REDACTED] backing out of trailer [REDACTED] in door 52 with a tall skid the top of the skid hit the door causing the door to roll down. Mr. [REDACTED] backed into the door and caused the door to pop out from the railing and caused the door rollers to pop out. One of the rollers hit Mr. [REDACTED] on the right side of the face beside his eye causing a laceration and bleeding. Reference [REDACTED] originally entered an injury
