On [REDACTED] [REDACTED] stated he woke up sore and called out sick from work without stating OJI. On [REDACTED] [REDACTED] was delivering to a customer and slipped on an ice patch [REDACTED]ing onto his back and knees. [REDACTED] reported this to dispatchers when back to the terminal and stated to HSSE he injured his back on [REDACTED] but the [REDACTED] today made it hurt more.
