What happened

EMPLOYEE OPENED THE WASTE WATER PIT TO PERFORM A REPAIR ON A BROKEN PIPE. WHEN THE REPAIR WAS COMPLETED HE WAS CLOSING THE LID WITH THE PROPER CHAIN FALL AND WHILE HOLDING ONLTO THE LID HIS FINGER GOT A LACERATION BY THE LID.
