Patient kept attempting to get out of bed. I put him in [REDACTED]. I attempted to reorient him. He put his legs over the side rails. I attempted to reorient him again. He then kicked me in the face hitting my eye where my glasses were. He then said I will do it again. Patient kept trying to get out of bed again.
