Before the incident

Removing an 8 inch chuck from the spindle of a CNC lathe

What happened

The operator neglected to use a support bar inside of the chuck spindle bore and or use the available and portable chuck removing crane when the chuck was unscrewed from the last thread the chuck dropped slightly and momentarily pinched the operators hand between the chuck and a part unloader actuator valve located a few inches below the spindle.
