AS EMPLOYEE WAS ATTEMPTING TO MAKE A DELIVERY HE NOTICED A DOG STARTING TO COME AT HIM AND ATTEMPTED TO BITE HIM SEVERAL TIMES AS HE RETREATED AND [REDACTED] BACK TO HIS TRUCK HE FELL ON THE STAIRS OF THE CUSTOMERS RESIDENCE AND LANDED ON HIS LEFT HIP CAUSING A CONTUSION AND A CUT HE IMMEDIATELY NOTIFIED MANAGEMENT AND WAITED FOR RELIEF BEFORE HEADING TO THE CLINIC FOR FURTHER EVALUATION
