What happened
The employee took a tool out of the spindle placed in the holder on the side of the machine placed an Allen wrench in the tool and then pushed on the Allen wrench and it let go slipping out of the set screw causing [REDACTED] arm to graze the tip of the end mill. This resulted in an abrasion on the inner part of his arm. A manager administered first aid on site and then transported [REDACTED] to the urgent care clinic.
