WHEN EMPLOYEE COULDN 'T REACH THE LAST CASE EMPLOYEE STOOD ON A BOX IN THE A LOCATION THAT COULD NOT HOLD EMPLOYEE WEIGHT THE BOX COLLAPSED AND EMPLOYEE HIT THE LEFT SIDE OF ARM AND TORSO ON THE PALLET.
