Root CausesTraining Associate was not trained on how to properly handle despool material. Despools need to be overwrapped to help eliminate excessive material and pulling. Handling Method Associate was handling an excessive amount of material. When handling the material the material folded due to being light gauge. Equipment Design The current equipment has the associate enter the area to hand feed the strip to the thread table. Corrective Actions Safety Stand down with all shifts CompleteCommunicated the importance of wearing PPE properly CompleteDevelop a JSA on overwrapping despool material. This will then be added to the training guide for Line Operators and Assistant Operators.
