What happened
Upon video review and associate questioning it was determined that the associate had a habit of supporting the first pieces of material that run onto the skid. The reason for this habit was to prevent the material from ripping damaging the paper that lays across the deck boards of the skid. After these initial pieces have the proper placement on skid the associate is able to run the rest of the material onto the skid without issues.Root Cause1 Poor work habits by the associate to introduce a part of his body to a moving strip while the line is running.2 No defined method for how to handle initial cut to length pieces on skid to ensure correct orientation.Corrective Actions1 Coached and reviewed with all associates on SPP 14.0 Best practice is not to touch a moving strip.. Complete2 Create a Talladega specific JSA that details out how to properly place the first blank in the correct orientation on the skid before running remaining material onto the skid.
