What happened

1.Associate did not want to tear up a bunk in the rear. This caused the material to be side loaded as opposed to rear loaded. Rear load would have put the skids on at a 42 width on the deck vs. 86 width if side loaded leaving 27 per side to move around for securement.2.There was already a truck in the rear load dock which would have caused the driver to wait.3.Associate pulled the drings completely out as opposed to standing them up prior to the skids being placed on the deck. This would have allowed side to side securement from the ground.4.Driver did not think of an alternate method to secure from the ground X the straps.5.Insufficient review of primary driver safety policy [REDACTED]
