What happened

The associates foot placement combined with the slippery conditions of the catwalk resulted in the loss of his balance traction grip. The associates left foot was too close to the edge of the catwalk as he took his step. Once he applied his weight on his left foot it lost traction and slipped off of the catwalk causing him to fall between the catwalk and the tractor trailer.
