on [REDACTED] [REDACTED] employee was seen at site first aid for pain in his shoulder while working loading 10 # castings into the ENSHU on SRW treated and sent back to work. 6 1 again stated his r shoulder was hurting and left work early to seek treatment but did not. returned to work 6 2 left early and finally was seen at urgent care. Given restrictions. on 6 3 was sent to panel physician and diagnosed with R cervical and shoulder inflammation.
