Employee reported that her cargo van was hit by another vehicle from the side by another car on [REDACTED] around [REDACTED] as she was driving to the next delivery location on [REDACTED]. Employee was examined by the SBH Health System Bronx [REDACTED] for a check-up and was discharged after the initial assessment was completed on [REDACTED]. Hospital provided employee with detailed documentation and advised that the driver is good to return to work on [REDACTED]. On [REDACTED] employee visited CitiMed Bronx [REDACTED] and was advised not to perform her delivery driver job responsibilities for 3-4 weeks and undergo physical therapy. Employee was offered light duty on a condition her doctor can assess and provide a letter for Light Work.
