EE was setting up the filler to perform a swab test and was running hot water through the system

What happened

When the operator removed the cover to the pipe that was to be swabbed there was an unusual amount of steam and hot water the operator attempted to lace the cap back on when the heat and pressure build up caused the cap to blow off before he could place a clamp on it.
