Before the incident

On the morning of [REDACTED] at [REDACTED] an operator mobilized to adjust the valve on the WAS wasting tank.

What happened

The valve is on an elevated concrete skid. The change in elevation is between 20 and 23 inches above ground surface. There is a small step stool that is used to access the skid. After completing the adjustments to the valve the operator attempted to dismount from the skid and ended up rolling his left ankle underneath him.
