What happened

Instead of securing the tool in a vice and using the cut-off wheel the employee attempted to remove the locked-up nut by holding the tool against a 30-inch grinding wheel. The grinding wheel caught the tool and pulled it bringing the employee 's hand into contact with the wheel.
