Before the incident

Removing a piece of glass from a crate.

What happened

[REDACTED] was working in the shop attempting to remove a piece of glass from a crate to place onto a panel. While using the gantry crane [REDACTED] was positioned very close to the wooden crate. A splinter of wood approximately one inch long was protruding from the crate. As [REDACTED] brushed up against the crate the splinter punctured his stomach on the left side above the belly button. The crate was above waist height requiring him to reach over and bend while operating the crane.
