Before the incident

EE entered the C3 roll pack without locking out the machine and he did not inform other employees he was entering the machine. EE admitted he was taking a short cut by not locking out the machine. EE did not inform other employees he was entering the machine. This incident could have been prevented if he would have followed proper LOTO protocol.
