What happened

Operator was performing 60 headrest at station [REDACTED]. When she attempted to dumb the pallet back the lock mechanism to hold the frame onto the build pallet failed. She attempted to catch the seat with both arms and her leg. She told her supervisor that it happened and no incident report was filed. She claimed it was not hurting at the time. When she came back the next day she wore a brace and claimed that her right arm was hurting.
