Before the incident

Employee was hoisting hydro blasting equipment with carry deck crane.

What happened

While lifting hydro blasting equipment out of the tank being cleaned the nozzle became clogged and the water pressure caused the cleaning equipment to swing out of control hitting the side of the crane boom and then coming through right side window striking employee in the right hand.
