Before the incident

TM was moving a tooling plate with a fork truck preparing to load it. The TM stopped exited the fork truck and began loading tooling onto the top plate (with the plates still on the forks raised approximately 3-feet off the ground).

What happened

When he picked up the plate off of the stack of plates he did not realize that the plate he picked up with the fork truck was actually two plates stuck together one on top of the other. During loading the bottom plate came loose and fell on his left foot resulting in a fracture.
